Initial Comments:
Per receipt of correspondence dated December 7, 2023 requesting closure of the above location as an ICF/IID residence, a closure survey visit action has been completed for this facility on December 15, 2023. The purpose of this visit was to discontinue participation under 42 CFR, Part 483, Subpart I Regulations for Intermediate Care Facilities for Individuals with Intellectual Disabilities. The total number of beds currently certified under the location totals four.
All applicable materials have been forwarded to the Central Office of the Division of Intermediate Care Facilities for processing to the Office of Long-Term Living, Department of Human Services. This action will be effective December 1, 2023, the date requested in the provider correspondence.
